- The Memory Number® test is designed to give individuals an understanding of their memory health.
- It is a scientifically validated test, often utilized in memory centers and doctor’s offices, used as a general screening tool to assess memory and cognition.

You may take as long as you need, however, the assessment usually takes 15-20 minutes.
The Memory Number® – A free, self-administered test that offers a baseline understanding of brain health. This written test evaluates how your memory is functioning and assigns a number, 0 to 22, and can be easily shared with family members or a physician.
Immediate Results – A staff member will grade your test as soon as you finish. Depending on your score, we will assess if a follow-up appointment is suggested.
Return Annually – We suggest you return once annually to track your scores over time.

Why do I have to fill-out a health questionnaire?
Many things can affect normal thinking including medications, poor general health, or mental health disorders. The interpretation of your test will be more accurate if the health information requested is available to the interpreting doctor.
Can I enroll in a research trial designed to prevent Alzheimer’s Disease?
Yes. Our screening process has changed from years past. Anyone interested in our FDA approved trials will go through a sponsor approved screening process. This screening process typically consists of a questionnaire, physical, and lab work, but may have other criteria to meet. If I do poorly today on my test, does it mean I have Alzheimer’s Disease?
No. There are many reasons why a person may not perform well… nervousness, poor sleep, medication side effects, or poor concentration are just a few of the many correctable causes of a poor test result.
Why do I have to sign a legal disclaimer?
It is important to know that this test should not be used as a diagnosis. Alzheimer’s and Dementia have several factors that must be evaluated by a Physician. A good score can offer some reassurance while a less than expected score can encourage you to seek further evaluation. You will not be meeting with a doctor and a doctor-patient relationship is not being established. Legally, the Columbus Memory center needs to be perfectly clear that this testing cannot replace a thoughtful conversation with your doctor. If you feel you need further evaluation, please request an appointment.
