New Patient Information

Becoming a New Patient:

In order to become a patient of Dr. Liss you must be referred by your Primary Care Physician with one of the following diagnoses:

  • Mild Cognitive Impairment
  • Dementia
  • Alzheimer’s Disease

By concentrating on these diagnoses, Dr. Liss has more time to spend researching and giving the highest quality care to each of his patients.

New Patients:

New patients will be required to fill out this New Patient Information packet. Please be sure to bring the following to the appointment:

  • Insurance Cards
  • Photo ID
  • Covid-19 Vaccination Card
  • All current medications (prescription and over the counter).
